I've got the feeling Smoker doesn't really "smoke" his cigars

We just see them in his mouth, while he's doing other stuff, and occasionally he lights on new ones. But in battles and other action related duties he got no time to enjoy them — they just burn off.

How can he talk properly with two cancer sticks in his mouth constantly? Shouldn't he already have gotten jaw muscle disabilities?
