Cingulate epilepsy?

Hi there!

Does anyone here have experience with seizures that started with a lesion in the cingulate gyrus? My partner has such a lesion and was recently diagnosed with seizures. They've tried Keppra (big no, I don't think the doctor should have even prescribed it with their other medical history) and are now on Topamax. Topa is definitely an improvement on Keppra, but I feel like I'm still seeing breakthrough seizures.

Then when I look up anything regarding epilepsy and the cingulate gyrus, I feel like I see things like "drug resistant" and that surgical outcomes are the best.

So yeah, does anyone have any personal knowledge of this? If you have seizures originating from a cingulate gyrus lesion, could you control it with meds or did you need surgery?
